8/9/2023 EnchantaVaughn Approved Upon arrival to the summer program, the client appeared to be in a content state as evidenced by greeting QMHP-C with moderate emotions. During morning socialization activities, the client remained focused and sat quietly in his seat while eating breakfast. During midday socialization activities, the client exhibited difficulty following directions given and utilizing respectful words with adults while outside on the playground. During afternoon socialization activities, the client interacted with his peers in a positive manner while cleaning up after lunch. Arrival to the client’s QMHP-C provided the client with verbal praise and encouraged him to continue remaining focused and sitting quietly in his seat. During midday socialization activities, QMHP-C redirected the client and reiterated the importance of following directions given and utilizing respectful words with adults. During afternoon socialization activities, QMHP-C provided the client verbal praise and encouraged him to continue interacting with his peers in a positive manner. The client responded well towards the summer program as evidenced by participation and engagement. During morning socialization activities, the client agreed to continue to remain focused and sit quietly in his seat while eating breakfast. During midday socialization activities, the client was unreceptive to redirection by crying and saying, “I want my mommy”. The client was then able to calm down and agreed to follow directions given and utilize respectful words with adults while outside on the playground. During afternoon socialization activities, the client agreed to continue interacting with his peers in a positive manner while cleaning up after lunch.
